Telehealth Disclaimer

Not a medical provider

Platform Health is not a medical provider. Clinical services are provided by independent licensed providers. Not for emergencies — call 911.

Who provides your care

Clinical services in this program are provided by an independent medical group. The medical group is not owned or controlled by your clinic or Platform Health and makes all diagnosis, treatment, and prescribing decisions.

Before your visit you see the medical group’s own telehealth consent and its Notice of Privacy Practices, in its own visit flow. Telehealth programs are available only in states where a licensed provider can care for you, for the place you will be during the visit.

Nothing is guaranteed

Medical evaluation required. Treatment and prescription approval are not guaranteed and are determined solely by an independent licensed clinician based on medical appropriateness.

If the clinician decides treatment isn't right for you, you won't be charged for medication.

Your pharmacy, your choice

You can fill any prescription at the pharmacy of your choice.

Emergencies

Not for emergencies. Call 911, or call or text 988 for a mental health crisis.
