• The indications, benefits, and reasonably foreseeable side effects.
• Alternative treatments, including no treatment.
• The clinic of medicine is not an exact science; results cannot be guaranteed.

I agree to follow my provider's instructions and to communicate any concerns or symptoms via the secure messaging system.
