• The benefits include increased access to care without travel.
• The risks include the possibility that information transmitted electronically may not be sufficient for proper decision-making.
• You retain the right to receive in-person care; we will refer you for in-person care when appropriate.
• Your information will be handled in accordance with HIPAA and applicable state laws.
• You may withdraw your consent at any time.
